Personal Data Inventory - When your first appointment is scheduled, a PDI
form will be mailed to your home. It is required that this form be filled out
completely and brought to your first session. This form is a vital part in the
communication between you and your counselor.
*Our Fees - Each session is $55.00. Payment is expected at the
time services are rendered. Checks, Cash, Visa & Mastercard are accepted.
Cancellations Without
Notice - If appointments are not
canceled at least 24 hours prior to the appointment, you will be responsible
for a $30.00 cancellation fee for day-time appointments, $55.00 for evening
appointments. This payment will be collected prior to any
future sessions.
Returned Checks - You will be responsible for a $30.00 bank charge
for any returned checks. This payment will be collected prior to any future
sessions.
Evening Appointments - Evening hours are limited and should be valued by
all clients. Due to the high demand for these appointments, if you cancel more
than 2 evening appointments, future appointments will be scheduled during
9:00am - 5:00pm office hours.
